Verification of Benefits for Health Insurance

Currently, Los Angeles County Addiction Treatment Center accepts a variety of PPO health insurance plans on an out-of-network basis. For those with private, non-governmental (i.e. not Medicare or Medicaid/Medi-Cal) PPO health insurance plans, we provide the form below in order to get a head start on verifying your benefits. These types of plans are usually provided by an employer, or purchased through the Healthcare Marketplace (aka: ObamaCare) and are purchased either through the main federal website (www.healthcare.gov) or in some cases, through individual state exchange websites in cases where there state offers it. In California, for instance, it would be www.coveredca.com.

Upon submission, our Verification of Benefits Team will immediately get to work and have detailed coverage information available for you within a very short time. In some cases, the cost of treatment may be fully or near fully covered. In other cases, there may be an initial deductible or co-payment required prior to coverage kicking in. We will have all of the details available for you and gladly answer any and all questions you may have.
